Behavioral assessment is an important aspect of the development of a behavior program.  In many states it is a legal mandate that a Functional Behavior Assessment (FBA) be completed by a qualified person prior to the implementation of a behavior support plan.  Again, users of this site are welcome to download and use the below documents for their own uses and modify the documents as necessary.

Direct Observation

The Direct Observation Form is useful when doing just that, directly observing behavior.  I have this form set up in a way so that an experienced observer can complete both A-B-C data and frequency data at the same time.  It is recommended that prior to implementing a new behavior program, direct observation be done in at least two distinct environments.

Functional Assessment Interview

Completing a Functional Assessment Interview (FAI) is a good first step when taking on a new behavior case.  Though the assessment is dependent on anecdotal information from other people who have experience with the person, it will give you a better idea of the challenges facing the person and/or team that ultimately led to the referral for your services.  The typical duration needed to complete an FAI is anywhere from 45-90 minutes, however, if you have a group of people who like to talk, depending on how useful their stories are, you can get a lot of information, though it could take a while.

Learning Styles Assessment

I honestly have only completed this assessment one time since I wrote it.  The first part of the assessment is from the work of another person (I will find out and cite this person) and the second part was my best attempt at isolating distinct learning style information.

Positive Environmental Checklist

This is a very useful assessment when trying to figure out if there are environmental variables that are either directly causing the person's problematic behavior or interfering with the approaches in the behavior program.

Preference Assessment

I would recommend that you have one other person help you complete this assessment.  Typically utilized for children and severe or profound ID adults, this assessment can give you information regarding the person's preferred tangible reinforcers.

Self-directed Functional Assessment Interview

Sometimes a person of importance, i.e. Mom, Dad, previous teacher, etc. is unable to make a meeting when you are conducting the FAI.  If this occurs you can send this assessment to that person and still be able to get their input and knowledge regarding a specific person's behavior.
